I HEREBY APPLY to be registered with Basketball South Australia for the 2022 NBL1 Central season. I acknowledge that the following information is kept by BSA to meet its obligations and to ensure that I can be contacted when required for Tribunal or Integrity related matters and also ensure the information displayed on all of our platforms is accurate.
BEFORE BEGINNING THIS REGISTRATION YOU MUST HAVE THE CORRECT "TEAM CODE". This code is unique to your club and is an 8 digit number which is different for each men's and women's team in the league. Without this code, you cannot continue and an incorrect code will not register you to your team. Contact your club for this number.
To be eligible for Round 1 this form must be completed by all players by 5 pm Monday 4th April 2022.
For persons under the age of 18 at the time of registration, this form must be completed by a parent/guardian. Payment is only required for current player membership if not previously paid for season 2022.
